We believe you should be able to live safely in your own home for as long as possible. To make this happen, Lynn Community Health Center has partnered with Greater Lynn Senior Services to create a joint venture called Element Care.
A “Health Center on Wheels” PACE is designed for seniors (55+) who might otherwise need a nursing home but want to stay independent. Your care is led by your LCHC provider, but it extends far beyond the clinic walls.
We provide a complete range of services—preventive, primary, acute, and long-term care. You can receive medical services in your own home, or you can visit one of the PACE Adult Day Health Centers for meals, social activities, and medical check-ups.

PACE Locations: PACE centers are located on School Street and Market Street in Lynn, as well as in Beverly, Gloucester, Methuen, and Lowell.
