Lynn Community Health Center’s Community Support Program connects patients and community members with practical guidance, resource navigation, education, and non-clinical support from people who know the communities they serve.
The CSP team is made up of trusted front-line public health professionals who live, work, and build relationships in the communities around LCHC. Their work helps people understand available resources, navigate barriers, and move toward better health with support that is grounded in lived experience and local knowledge.
What Community Support Programs Do
Community Support Program staff help bridge the gap between health care, community resources, and the everyday challenges that can affect a person’s well-being.
The team provides:
- Practical guidance and education
- Help navigating community resources
- Non-clinical support for people facing barriers to care
- Advocacy for vulnerable and underserved populations
- Support informed by shared wisdom and lived experience
- Connections that help close inequities in health care and resource access
