Connecting patients and community members with support throughout our community.

Lynn Community Health Center’s Community Support Program connects patients and community members with practical guidance, resource navigation, education, and non-clinical support from people who know the communities they serve.

The CSP team is made up of trusted front-line public health professionals who live, work, and build relationships in the communities around LCHC. Their work helps people understand available resources, navigate barriers, and move toward better health with support that is grounded in lived experience and local knowledge.

What Community Support Programs Do

Community Support Program staff help bridge the gap between health care, community resources, and the everyday challenges that can affect a person’s well-being.

The team provides:

  • Practical guidance and education
  • Help navigating community resources
  • Non-clinical support for people facing barriers to care
  • Advocacy for vulnerable and underserved populations
  • Support informed by shared wisdom and lived experience
  • Connections that help close inequities in health care and resource access

The CSP Team

LCHC’s Community Support Program brings together a diverse team of front-line public health professionals, including:

  • Community Health Workers
  • School-Based Community Health Workers
  • Care Coordinators
  • Recovery Coaches
  • Recovery Support Navigators

Together, this team supports patients and community members with compassion, trust, and an in-depth understanding of local barriers, resources, and paths to wellness.

CSP staff are not only familiar with the communities LCHC serves. They are part of them.

Because they live, work, and build relationships locally, CSP team members bring a unique understanding of the barriers people face and the connections that can help. Their work is centered on trust, advocacy, and practical support for people who may otherwise have difficulty accessing care or community resources.

If you’re facing food insecurity, community meals and local food pantries are available to help. Community meals are open to everyone. Local pantries may have different requirements; some ask for ID or a recent piece of mail, while others are open to all.
