Senior Care Options and OneCare

Lynn Community Health Center participates in two programs through MassHealth which coordinate comprehensive primary care and social support for patients and families.

If you are interested in either of these programs, please talk to your care team. We can help you determine if you are eligible and assist with the enrollment process.

Senior Care Options (SCO)

  • For patients age 65+, enrolled in Medicaid and Medicare
  • Coordinates primary care, geriatric specialty care, and social support
  • Provides respite care for families and caregivers
  • No co-pays
  • No cost for transportation to medical appointments
  • 24-hour access to your care team
