What Is a Community Health Worker?
Community Health Workers are trusted members of the community who help patients navigate health care and social service systems. They are often the first to notice a need—and the first to find a way forward.
CHWs support patients in many ways, including:
- Helping people access housing, food, or transportation
- Assisting with health care follow-ups and referrals
- Offering coaching, advocacy, and health education
- Supporting behavioral health and substance use recovery
- Conducting outreach and helping patients stay connected to care
CHWs work closely with providers, case managers, and other team members—but they bring something unique: lived experience, cultural knowledge, and a deep understanding of what it takes to get help and stay connected.
Make an Impact in the Lynn Community
As a CHW at LCHC, you’ll work directly with youth and families in Lynn—especially those who need extra support managing medical, social, or emotional needs. Your role may include:
- Educating and coaching young patients and families
- Connecting people with local services and community resources
- Helping arrange and sometimes attend medical appointments
- Re-engaging patients who’ve missed care
- Participating in community outreach
Who Thrives in This Role?
This role is ideal for people who are:
- Compassionate and resourceful
- Familiar with the Lynn community
- Comfortable talking about mental health, addiction, and recovery
- Organized, detail-oriented, and team-focused
- Ready to support youth and families through real-world challenges
Preferred qualifications:
- 3+ years of experience in community health, outreach, or case management
- CHW certification (or willingness to obtain it)
- Valid driver’s license and access to transportation
- Bilingual or multilingual skills highly valued
”The Community Support Program at LCHC is a diverse team of front-line public health professionals who are trusted community members, experts at resource navigation, and committed advocates for the most vulnerable and underserved populations. Comprised of Community Health Workers, School Based Community Health Workers, Care Coordinators, Recovery Coaches, and Recovery Support Navigators, the CSP team provide practical guidance and education, non-clinical support, and shared wisdom of lived experience with the hope to close inequities in health care and resource distribution. CSP staff live, work, and exist within the communities in which we serve, providing a unique and in-depth understanding of barriers, connections, and bridges to wellness.
