Lynn Community Health Center is dedicated to providing excellent healthcare for our community. We choose to seek accreditation from several nationally-recognized organizations in order to maintain the quality of our work at the highest standard.
Accreditation involves regular, on-site inspections of our safety environment and reviews of our patients’ health outcomes and satisfaction metrics. Provider and staff qualifications and training requirements are also reviewed.
This health center received HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Our Health Center Quality Leader Awards
Health Center Quality Leader badges are awarded annually by the Health Resources and Services Administration to health centers across the country. Criteria is adjusted each year to ensure each award is meaningful as clinical and community services improve across the spectrum. Badges are awarded based on the prior year’s Unified Data System (UDS) data from each organization.
Additional or prior badges awarded to LCHC include “Addressing Social Risk Factors,” COVID-19 Public Health Champion,” and more.
The National Quality Leader – Heart Health badge is awarded to health centers that meet or exceed three of the four measure targets, which are based on the Million Heart Goals. Learn more.
The Improving Health Care Access badge is awarded to health centers that meet two requirements: (1) they must either earn a National Quality Leader or Health Center Quality Leader badge, or improve by at least 15% in one or more Clinical Quality Measures in back-to-back UDS reporting years; and (2) they must also show at least a 5% increase in back-to-back UDS reporting years for either total patients or for patients receiving mental health, substance use disorder, vision, dental, or enabling services. The Advancing Health Information Technology (HIT) for Quality badge acknowledges health centers that successfully fulfill all prerequisites to enhance HIT services. To qualify, health centers should satisfy five essential criteria: implementing an electronic health record (EHR) system, providing telehealth services, electronically exchanging clinical information with crucial healthcare providers and settings, actively involving patients through health IT, and collecting data on patient social risk factors. A Patient-Centered Medical Home approach emphasizes comprehensive, coordinated, and patient-focused care, often involving a team of healthcare professionals who work collaboratively to provide personalized and accessible healthcare services.
