Celebrating Excellence in Clinical Quality

We are pleased to share that our health center recently received multiple Community Health Quality Recognition (CHQR) badges from the Health Resources and Services Administration (HRSA)—a milestone that underscores our deep-rooted commitment to remarkable patient care.

Honoring Quality Across Every Dimension of Care

This year, we proudly earned the following CHQR distinctions:

  • Advancing Health Information Technology for Quality – A badge we’ve held since 2021, awarded for our robust telehealth services, secure exchange of clinical information with key providers, engaging patients via digital tools, and proactively collecting health-related data.

  • National Quality Leader for Heart Health – A new distinction recognizing our performance in critical areas: tobacco use screening and cessation, aspirin or antiplatelet therapy for ischemic vascular disease (IVD), statin therapy for cardiovascular disease, and controlling hypertension. We placed in the top 20% nationally on three of these measures.

  • Improving Healthcare Access – Also a first for us, this badge highlights our success in increasing patient access to services—illustrating how our commitment to removing barriers is paying off.

What These Recognitions Mean

CHQR badges honor health centers like ours that demonstrate measurable improvements in access, clinical quality, health outcomes, and use of health information technology, as reflected through the Uniform Data System (UDS) data. Their annual awards are based on the most recent UDS reporting year and reflect a center’s sustained quality gains.

These achievements are far more than accolades—they’re a reflection of our philosophy: that exceptional patient care is built on a foundation of teamwork. From clinicians to front-desk staff, IT professionals to support services, every team member plays a role in advancing quality.
