PACE

Lynn Community Health Center, along with Greater Lynn Senior Services, has established Element Care as a joint venture to bring Program for All Inclusive Care for the Elderly (PACE) to frail seniors in our community.

Element Care PACE is a personalized healthcare program that provides medical services to adults 55+.  The Element Care PACE program will give you the support you need to remain safely in your own home for as long as possible. Your PACE team, led by your primary care provider at Lynn Community Health Center, provides a comprehensive range of preventive, primary, and acute services.

Services include:

• Complete medical care from a team of healthcare professionals, including therapists, psych nurse practitioners, and other medically necessary services, including dental, vision, hearing and more
• Transportation to scheduled medical appointments
• Adult day health program for breakfast, lunch and activities or continue going to your own ADH for free.
• At home medical check-ins, personal care, and meal support

Element Care PACE services the North Shore & Merrimack Valley with PACE centers in:

• Lynn, Beverly, Methuen, Lowell & Gloucester
• If you are on Medicare and income eligible, there are NO monthly premiums, co-pays or out-of-pocket expenses to enroll in Element Care PACE.
