What is Integrated Primary Care?

At Lynn Community Health Center, integrated primary care isn’t a new initiative — it’s the foundation of how we care for our community. For decades, we’ve brought together medical and behavioral health services under one roof, ensuring that individuals and families receive coordinated, compassionate care that meets their full range of needs.

Integrated care at LCHC means our patients don’t have to navigate their health alone. A patient managing diabetes might also receive support for depression from a therapist just down the hall. A child coming in for a routine check-up might also receive a vision screening and a referral to a pediatric eye specialist. Pregnant patients may receive prenatal care, dental support, and behavioral health counseling — all as part of one unified care team.

This approach extends across our services. Our dental and eye care teams are part of the full picture of primary care at LCHC, working hand-in-hand with medical and behavioral health providers to ensure no part of a patient’s health is left out of the conversation. Our Community Support Programs team rounds out this model by helping patients stay connected to their care with appointment reminders, resource referrals, follow-up calls, mailed letters, text messages, and more. In 2024 alone, they completed almost 29,000 patient touches, strengthening trust and connection every day.

The impact of this model is seen in the thousands of people we serve each year. For example, in just 2024:

  • We delivered 65,146 behavioral health visits to 6,032 individuals

  • Our dental team completed 47,807 visits for 13,485 patients

  • Our eye care department saw 6,880 patients in 8,738 visits

  • And our OB/GYN team supported families in welcoming 655 new babies into the world

These numbers are more than statistics — they’re a reflection of the depth, breadth, and heart of the care we provide. Integrated primary care is what allows us to respond to the real-life challenges our patients face and to meet those challenges with a team-based, person-centered approach. Each of these numbers is a real patient, a real story, a real community member, a real family.
