Innovation Award – Access Model 2.0
The innovation award is given to a person or team for designing and/or implementing a process, system or concept that substantially improves one or more of the following areas: Patient Access; Patient Experience; Employee Engagement; Health Equity; Financial Performance; or Strategic Positioning
The Access Model 2.0 initiative represents a bold reimagining of how care is delivered at LCHC, centered on equity, compassion, and patient connection. Led by the Blue, Purple, and Green Teams, this innovation has transformed clinical operations to expand access, improve the patient experience, and strengthen financial sustainability. Through thoughtful redesign, the teams increased the number of 30-minute visits for complex patients, reduced no-show rates, and created a more efficient, relationship-based model of care. Beyond meeting these goals, Access Model 2.0 has deepened collaboration across providers, nurses, and MAs, improved work-life balance, and enhanced continuity of care. Patients now benefit from more time, attention, and support, both inside and beyond the exam room. This initiative stands as a powerful example of what is possible when creativity, teamwork, and mission-driven innovation come together to advance health equity and excellence in community care.
